INSTRUCTIONS
If you live in Utah, here are some simple instructions to get your background check done. If you do not live in Utah, let us know and we will provide you with instructions for your state.

  1. Download and fill out the top section of the following form:

  2. Take your completed form with a valid picture ID and the $15 processing fee to: 
  3. Utah Bureau of Criminal Identification
    3888 West 5400 South
    Taylorsville, UT 84129
     
  4. Show your ID, have the clerk verify your ID and add your fingerprints to the application, and pay the fee. 
  5. Wait a few minutes and you will receive an official criminal history report with the official State of Utah embossed seal. 
  6. Bring the original, completed Criminal History Report to volunteer orientation or mail to:

*Utah BCI Hours: Monday-Friday 8am-5pm, closed weekends and holidays

*If you cannot make it to the office, forms and payment may also be mailed to this address, but you will need to visit your local police department with your valid ID to obtain fingerprints before mailing the form. You will also need to submit the Third Party Release Form found here: http://site.utah.gov/dps-criminal/wp-content/uploads/sites/15/2015/01/Third-Party-Release-Form.pdf

The third party release form will require the following information:

Print, sign, and date the form. Mail it to: Utah BCI, 3888 West 5400 South, Taylorsville, UT 84129

Background Check FAQ
 

Why are background checks mandatory?
For the safety of our campers and volunteers, we background check all of our volunteers who work with children. There are no exceptions to this policy.

What do I need to fill out on the form?
Fill out the top section of the form only. The following information is required:

  • Name
  • Date of Birth
  • Previously Used Names
  • Mailing Address
  • Physical Address
  • Contact Phone Numbers
  • Social Security Number
  • Driver’s License Number
  • Physical Description
  • Applicant Signature

Who will see my background check report?
Only the co-directors of the camp are authorized to review and file your background check information. All information will be kept confidential and only be used to verify that you do not have any crimes against children. The co-directors of the camp are Amy Stocks and Hillary McDaniel.

Is there a cost?
Yes. There is a $15 processing fee assessed by the state of Utah. This fee is non-refundable through RRCFG-SLC. If you cannot afford to pay the fee, please email us immediately and we can discuss options. You will also need a stamp to mail your background check report to RRCFG-SLC.

I can’t get it done before the deadline. What do I do?
E-mail us immediately. You cannot volunteer with us without having a criminal background check completed.

Do I have to do this every year I volunteer?
Yes - we need an updated form every calendar year.

I live outside of Utah. Where do I go to get a background check?
E-mail us ASAP if you need instructions on obtaining a background check outside of Utah.

What if I don't pass the background check?
We will notify you and handle the issue confidentially. We are only looking for violent crimes or crimes against children - other minor offenses will be simply ignored.
